Cronulla Water Polo Club members are covered under the Water Polo Australia Risk Protection Program, specially designed to meet the needs of Water Polo in Australia.

What is covered

The following insurance policies have been designed to protect Water Polo Australia members when involved in activities that have been sanctioned by the relevant Water Polo State or Territory Association.

Activities covered include competition, training, social functions, committee meetings, and fundraising, as well as travel to and from these activities.

Please refer to the policy wording for full details of the policy benefits, exclusions, terms and conditions.

The Water Polo Australia Insurance Program provides a “standard level” of cover for all members and has not taken into account an individual’s financial needs or circumstances. Water Polo Australia encourages all members to purchase their own Private Health Insurance, Life Insurance and Income Protection to ensure an adequate level of cover is in place and is based on personal circumstances.

How to make an insurance claim for injury / accident

If you have suffered an injury or accident whilst participating in water polo or a water polo related activity (including training, competition, official event or other sanctioned activity), please follow the steps below to lodge a Personal Accident claim.

Step 1

In the first instance of an injury or accident please submit an injury report with the club:

Download the Water Polo Australia Claim Form:

Step 3

Complete the Water Polo Australia Claim Form, ensuring all details are true and complete. Please note, you will need to source the following information to submit along with your claim: 

  • Medical report completed by the main doctor, chiropractor, physio or dentist who is providing treatment for the injury.
  • Employers statement and retorn to work statement (if claiming net loss of income benefit).
  • Original receipts for all non-medicare medical expenses.
Step 4

Send the claim form to info@cronullawaterpolo.com.au to complete Part 7 - Declaration by an Authorised Office Bearer

Step 5

Send the completed claim form to asiapac.claims@sportscover.com

Please note the following when considering insurance claims
  • You are only able to claim for costs that are not covered by Medicare. For example: Ambulance, Physio, Dental, Chriopractic, Private Hospital Accommodation. The Health Insurance Act does not permit insurers to contribute to any charges covered or partially covered by Medicare, such as Doctors, Public Hospitals, Surgeons fees, X-Rays & Anaesthegiology.

  • Insurance claims are limited and excesses vary on medical expenses. Please ensure you read the Water Water Polo Australia Policy for a complete view of what to expect. 

  • Once a claim is lodged, the injured player/parent will liaise directly with the insurer. The club is not able to fast-track or interfere with this process.   
  • Claims cannot be settled by the insurer until all treatment relating to the injury has been completed, accounts paid and refunds from private health insurers have been obtained.
